Dual Enrollment. You will sign off indicating you understand these rules in the "Dual Enrollment Acknowledgment Form." 
(see item #30). 

Interested students must review this PowerPoint presentation. You will sign off stating that you watched this video when you complete the "Dual Enrollment Acknowledgement Form," (see item # 29.). PowerPoint updated 02.05.2025

Please download the CCSD Local School Agreement, fill it out, and save it to your desktop. To sign it, you will need to use the "draw" feature; everything else is fillable.

You must be logged into your CCSD computer (365 account)

for this to work. 

 

You will upload this document to the The "Dual Enrollment Acknowledgement Form" (see item #26 under the "Dual Enrollment Acknowledge Form" button). 

 

You and your parent must sign this form for it to be complete. If you are missing the signature on the last page, your application will be considered incomplete and you will NOT be offered a Friday DE meeting. 

Next, click on the form below and answer the questions, acknowledge the rules and requirements, upload your testing scores for KSU or GA Tech (if applicable), and upload your initialed and signed "CCSD Local School Agreement" in this form

to complete the form below. If the form does not open, you are not logged into your CCSD 365 account.

Dual Enrollment Funding.

You will remain with your regular counselor, but Dr. White and Ms. Hawk will help you develop your Dual Enrollment (DE) college schedule. They can be reached at

The DE counselor will reach out to you once you have correctly completed the necessary form.
