Please download the following document. After downloading, click "Enable Editing," fill out the form, and save it to your desktop. You will need to upload this form to the "DE Student Acknowledgment Form" located under item #26 (see below).

Make sure you are logged into your CCSD computer (365 account) for this process to work properly.

Both you and your parent must initial next to each rule and sign the form for it to be considered complete. If you forget to sign the last page, your application will be deemed incomplete, and you will NOT be offered a Friday DE meeting.

Next, you must complete the Dual Enrollment Acknowledgment Form.
​
Acknowledge the rules and requirements, and upload your testing scores for
KSU or Georgia Tech (if applicable).
Additionally, upload the initialed and signed "CCSD Local School Agreement" in the form that you just completed above.

To complete the following form, you must log in to your Cobb County 365 account.
